1. Sponsor

This giveaway is sponsored by nutritionOS, LLC (referred to herein as the "Sponsor"), located at 12123 Shelbyville Road, Suite 100 PMB #253, Louisville, KY 40243.

2. Eligibility

The giveaway is open to individuals who are 18 years of age or older at the time of entry. Employees, representatives, and agents of the Sponsor, as well as their immediate family members and household members, are not eligible to participate. The giveaway is void where prohibited by law.

3. Entry Period

The giveaway begins on 3/20/25 and ends on 3/26/25 at 11:59 PM EST (the "Entry Period"). Entries submitted before or after the Entry Period will not be eligible.

4. How to Enter

To enter, participants must complete the entry form on the Sponsor’s website at https://accountabilityoperatingsystem.com/video-1-the-problem. Entries that are incomplete, illegible, or fraudulent will be void.

5. Prize

One (1) winner will receive an accountabilityOS Certification valued at $499. The prize is non-transferable and no cash alternative or substitution will be offered. The winner must comply with all terms and conditions stated herein.

6. Winner Selection and Notification

The winner will be selected at random from all eligible entries received during the Entry Period. The drawing will take place on or about 3/27/24. The winner will be notified via the email provided on their entry form within 1 business day of the drawing.If the winner cannot be contacted, is ineligible, or fails to claim the prize within 14 days from the date award notification was sent, the prize may be forfeited, and an alternate winner may be selected at the Sponsor's sole discretion.

7. Odds of Winning

The odds of winning depend on the total number of eligible entries received.

8. Publicity

By entering this giveaway, participants agree to allow the Sponsor to use their name, likeness, and entry for promotional purposes in any media without further compensation, unless prohibited by law.

9. Limitation of Liability

By participating, entrants agree to release and hold harmless the Sponsor and its respective subsidiaries, affiliates, suppliers, and advertising agencies from any liability, injury, loss, or damage arising from participation in this giveaway or from the acceptance, receipt, possession, or misuse of any prize.

10. Privacy

The information collected from entrants is subject to the Sponsor’s Privacy Policy, which can be found at https://nutritionos.org/privacy-policy. The Sponsor will not share or sell the entrant’s personal information, except as necessary for the administration of the giveaway.

11. General Conditions

The Sponsor reserves the right to cancel, suspend, or modify the giveaway if any fraud, technical failure, or other factor beyond the Sponsor’s reasonable control impairs the integrity or proper functioning of the giveaway, as determined by the Sponsor in its sole discretion. The Sponsor reserves the right to disqualify any individual who tampers with the entry process or violates these Terms & Conditions.

12. Governing Law

This giveaway is governed by the laws of the state of Kentucky, without regard to its conflict of law principles.
